Kingston Centre which was amalgamated with other hospitals in 1995 to form the Southern Health Care Network, provides a range of health care and support services to older people, many of whom have complex needs, living in the Municipalities of Bayside, Monash, Kingston, Greater Dandenong, Casey and Cardinia.  

In July 2000 the Southern Health Care Network became Southern Health - the largest Metropolitan Health service in Victoria.

The Aged Care Program comprises four clinical sub programs and a research and education sub program.

  • Rehabilitation and Medical Services (RAMS)
  • Residential Services
  • Aged Mental Health
  • Community Services
    A wide range of specialised community assessment and support services.
  • Research and Education Research and Education
    There are three research areas within the Program:
    Aged Mental Health
    Geriatric Medicine
    Geriatric Neurology
    Each of these areas is directed by their respective Monash University Professor.

The Aged Care Program interacts with other Programs within Southern Health.  The Musculoskeletal, Neurosciences and General Medicine Programs are particular referral sources. Along with the Acute Rehabilitation Program, the Aged Care Program provides sub acute care for patients from the acute sector.

GOALS

The service aim is to improve function and independence where possible, then maintains client function for as long as possible. The Program aims to deliver quality care consistent with achieving ACHS accreditation status.
